About:

Alliance for Housing and Healing is a non-profit organization founded 35 years ago with a mission to mitigate the interrelated problems of homelessness and chronic diseases like HIV/AIDS. We have since evolved to become a major provider of human services in Los Angeles County, serving over 2,000 people annually. With an annual budget of $12M, we provide direct housing to chronically homeless persons and their families living with HIV/AIDS; connect homeless patients experiencing high barriers to acuity with permanent supportive housing and other wraparound services; and increase client access to care and support. By reducing chronic homelessness, we reduce HIV/AIDS viral loads and eliminate the disease for the next generation.

Job description:

An Intensive Case Manager (ICM) is responsible for assisting clients who are homeless and who have a chronic illness or physical disability in every stage of the housing stabilization process. Services are provided most often in the client’s home, and include intensive coordination and evaluation of the client’s needs, abilities, and progress in gaining access and maintaining health, mental health, benefits and housing stability.

Responsibilities:

  • Assist clients with gaining independence through life skills (financial management, employment, education, accessing social services, legal issues, housing, healthcare, personal hygiene and coping)
  • Develop, implement and document an individualized case management service plan with the client to address the needs identified in the initial DHS-approved psychosocial assessment
  • Coordinate across departments, agencies, partners and providers to resolve issues related to health and housing that jeopardize stability
  • Participate in regular staff meetings, staff training programs, supervisory sessions, quarterly program meetings, and accept the responsibility for aiding the development of positive team relationships
  • Adhere to agency policy, procedures and the professional code of ethics

Required qualifications:

  • Bachelors Degree in a Social Services related field
  • Bilingual English/Spanish
  • Experience in a case management or similar role
  • Experience with Microsoft Office Suite of products (Word, Excel, PowerPoint)
  • Background check clearance required
  • TB screening required annually
  • Valid California driver’s license, proof of auto insurance and access to a car with the ability to travel throughout LA county

Preferred qualifications:

  • Experience working with the homeless and diverse populations
  • Knowledge of HIV/AIDS-related issues
