Lead Supportive Service Coordinator

Under the direction of the Director of Permanent Supportive Housing, the Lead Supportive Service Coordinator will supervise staff and have a client caseload out of our Long Beach office. Client services will be provided off-site, most often in the client’s home, and include the assessment, intervention, and evaluation of the client’s needs, abilities, and progress. The Lead Supportive Service Coordinator will also oversee contract compliance and data reporting.

Principle Duties:

  • Supervision of Supportive Service Coordinator(s)
  • Conduct home visits and provide supportive services to ensure housing retention and improvement of health and well-being.
  • Conduct intake and assessment of clients’ needs and goals.
  • Develop and monitor progress of the individual service plan with the client and staff.
  • Link clients to resources that promote stability, including but not limited to, benefits counseling and advocacy, mental health and substance abuse services, child care, employment assistance, and community engagement opportunities.
  • Teach and model independent living skills.
  • Collaborate with Alliance Housing Manager to avoid client evictions by addressing property management concerns.
  • Participate in outreach efforts to identify clients, as well as expand service knowledge.
  • Oversee contract compliance activities, including data monitoring and reporting.

Qualifications:

  • Minimum of 3 years’ experience as a service coordinator or case manager with similiar population.
  • Master’s degree in Social Work preferred; Bachelor’s degree in Social Work/related field required.
  • Strong knowledge of the complexity of HIV/Aids-related issues, chronic homelessness, and co-morbidities, including mental illness, trauma, substance abuse, aging, and chronic health issues.
  • HMIS or other database management experience.
  • Familiarity with housing first and scattered-site permanent supportive housing models.
  • Excellent active listening and crisis intervention skills with ability to model good judgement.
  • Excellent interpersonal, written and oral communication skills.
  • Ability to manage multiple tasks and priorities, balancing provision of direct service with documentation and data management.
  • Proficiency in Microsoft OfficeSuite (Word, Excel, Outlook) and Internet.
  • Access to a car, current California driver’s license, and insurance, with good driving record.
  • Background check and annual TB screening required.
  • Bilingual English/Spanish preferred.
  • Supervisory experience preferred.
