West Hollywood Service Coordinator

Job Description:

The West Hollywood Service Coordinator will be working in the City of West Hollywood Aging in Place Rental Assistance and Tenant Relocation Assistance programs. He/she is responsible for assisting West Hollywood senior residents age in place by assisting clients in obtaining rental assistance as well as assist those West Hollywood residents being displaced through no fault of their own locate alternate housing.

Responsibilities

  • Conduct client intake and assessment to identify housing needs.
  • Develop and implement an individual housing plan.
  • Assist clients in completing applications for housing and financial assistance programs.
  • Provide housing location services.
  • Educate clients regarding neighborhood amenities, services, transportation and tenant rights and responsibilities.
  • Identify resources, refer and link clients to appropriate housing and other service agencies or community programs to meet the needs of the client.
  • Create and maintain a collaborative relationship with referral sources, landlords and other community partners.

Qualifications:

  • Bachelor’s Degree in social work or related social service field OR a minimum of three years’ experience in direct service as a Case Manager with seniors, homeless clients or clients with chronic illnesses.
  • Must have access to a car, valid CA driver’s license, proof of CA auto insurance, and ability to travel countywide.
  • Excellent communication skills, both verbal and written.
  • Well-organized and detail-oriented with the ability to manage multiple tasks while meeting deadlines.
  • Ability to work both independently and as part of a team.
  • Working knowledge of Microsoft Office.
  • TB screening required annually.
