Alliance for Housing and Healing is a non-profit organization founded 35 years ago with a mission to mitigate the interrelated problems of homelessness and chronic diseases like HIV/AIDS. We have since evolved to become a major provider of human services in Los Angeles County, serving over 2,000 people annually. With an annual budget of $12M, we provide direct housing to chronically homeless persons and their families living with HIV/AIDS; connect homeless patients experiencing high barriers to acuity with permanent supportive housing and other wraparound services; and increase client access to care and support.  By reducing chronic homelessness, we reduce HIV/AIDS viral loads and eliminate the disease for the next generation.

Job description:

We are seeking a part time (16 hours) Licensed Clinical Social Worker (LCSW) to work with clients to remove impairments that are barriers towards independent functioning with a focus on relapse prevention and monitoring clients’ overall mental health. The Licensed Clinical Social Worker will travel between the Residential Care Facilities for the Chronically Ill (RCFCI) group homes in L.A. to meet with the clients and work closely with the multidisciplinary team.  L.A. group homes are located in Elysian Park and the Pico/Crenshaw area.

Responsibilities:

  • Participation in the screening of clients to assess for appropriateness of placement
  • Participation in case conferences and creating a client services plan with the multidisciplinary team
  • Creating and monitoring relapse prevention plans for clients with a history of drug/alcohol use
  • Monitoring the overall mental health of each client
  • Referring, coordinating, and maintaining contact with outside substance abuse and mental health service providers
  • Meeting with each client as needed at a minimum of once per month
  • Implementing Title XXII requirements that refer the social service needs of the clients
  • Implementing Department of HIV and STD Program requirements that refer to the social service needs of the clients
  • Assisting/training the facility staff in meeting the mental health needs of the clients
  • Providing crisis intervention as needed
  • Collaborating with group homes’ Social Worker to coordinate client services

Minimum qualifications:

  • Current CA Board of Behavioral Science License, LCSW
  • Current California driver’s license and auto insurance
  • Reliable and available transportation
  • Experience with Microsoft Office Suite of products (Word, Excel, PowerPoint)
  • Background check clearance required
  • TB screening required annually
  • Valid California driver’s license, proof of auto insurance and ability to travel within L.A.
  • Excellent written and verbal communication skills
  • Ability to work independently and part of a team

Preferred qualifications:

  • Working knowledge of HIV/AIDS
  • Strong clinical background with experience working with chemical dependency and dual diagnosis populations
  • Bilingual English/Spanish
